I’m pushing for universal quantitative neuromuscular monitoring to prevent residual paralysis and need a device that’s accurate and easy under drapes… Any pros/cons on TetraGraph vs TwitchView vs GE NMT, especially with a Bair Hugger running? Do you reliably get TOF ratio >=0.95 before extubation, and how are you documenting and alarming when cases run past the 2-hour mark?
I’ve had the best luck with TwitchView under drapes: if I ‘calibrate before warming’ and tape the EMG electrodes lateral to the Bair Hugger hose, it stays stable and I reliably extubate at TOFr >=0.95 at the adductor pollicis. Small caveat: the GE NMT (AMG) drifts more for me with warming unless the thumb is free to move. @OP, are your arms usually tucked or can you still access the adductor pollicis?
TetraGraph’s EMG tolerates Bair Hugger; I baseline pre-warming, auto-trend. EHR timer at ‘110 min’ prompts >=0.95; do you use facial?
I run a tiny USB fan across a fridge-chilled unglazed terracotta tile set on low spacers, which creates a cool boundary layer without spiking humidity; for my rabbit, the tile sits inside a covered hide so it’s cool but not drafty — just don’t aim the fan at them… @l_clark88 have you tried pulling air under the cool surface instead of blowing at the animal?
