Looking at a 2022 NACOR extract in our data warehouse, one modifier consistently dominates rural CRNA claims. Which CMS anesthesia modifier — QZ or QX — appears most in critical access hospital submissions, and what informatics or practice factors do you think drive that distribution?
In our CAH audits, QZ wins by a mile; > submissions, and what informatics or practice factors do you think drive — we found Epic’s charge router defaulted to QZ unless every TEFRA checkbox fired, so a quick monthly crosswalk of supervision logs vs modifier rates caught and flipped QZ on cases that should’ve been QX. Are your CAHs in opt-out states, or does QX only climb where Method II is turned on?
QZ dominates in our CAHs — opt-out plus solo CRNA coverage; QX only bumps where Method II and on-site MDAs exist. We cut down wrong-QZs by forcing attending NPI and 7-step medical-direction attestation before QX posts; otherwise it pends instead of defaulting to QZ. @davidK56 > that should’ve been QX. Agree — but only when the attestation timestamp sits within start–stop; do your edits check that?
In my pulls, QZ shows up most in CAH claims — like a Roomba, it goes everywhere unless you put up a gate. We added a prebill rule that forces QX when a supervising NPI or documented direction exists and pops a hard‑stop prompt, “Is this truly independent?”, if those fields are blank. One caveat: sites with regular on‑site MD coverage flip more to QX, so are your provider rosters and shift schedules feeding the mapper or is your EHR still keying off empty fields?
Picking up on last week’s hydration chat: for CKD cats that ignore plain water, I’ve had steady success offering Purina Hydra Care in a saucer about an hour after meals and placing it by their nap spot; intake beat any fountain I’ve tried. If they’re on sodium-restricted plans or binders, I clear it with the DVM and note it in the reception-to-tech handoff so timing stays consistent; link: https://www.purina.com/cats/pro-plan/cat-supplements/hydracare.
Mostly QZ in our 2022 NACOR CAH slice, driven by the charge router defaulting to QZ whenever the supervising field is empty or the ‘direction’ attestation isn’t checked. We tightened it by auto-switching to QX only if a supervising NPI is present and concurrency <=4, then running a daily mismatch audit. @elena_smith73, do your QX counts jump on days when an attending rotates in?
